WellCare PFFS Provider Information about Eligibility and Benefits

ELIGIBILITY: A Medicare beneficiary can join a WellCare Private Fee-for-Service (PFFS) plan if he/she:

  • Has both Medicare Part A and Part B
  • Does not have End Stage Renal Disease at the time of application
  • Lives in one of the service areas for WellCare PFFS plans

In order to continue with the registration process the NPI # is a require field but If NPI number is unavailable, please enter Tax ID in both the NPI and Tax ID fields.

 

BENEFITS: WellCare Private Fee-for-Service plans for 2009 include WellCare Concert, WellCare Melody, WellCare Melody-Plus, WellCare Prelude, WellCare Quartet, WellCare Serenade, WellCare Sonata and WellCare Sonata-Plus. PFFS plans are not a Medicare supplement, (Medigap), Medicare Select or Prescription Drug Plan. In addition, PFFS plans can also offer benefits with predictable member cost-sharing on hospital and other physician services
